Leaders from the health care industry, education, government and workforce agencies joined together at the Summit to:

  • Identify and understand workforce competencies emerging from patient-centric primary care home pilots.
  • Forge alliances between education and industry to guide the transformation of health profession education and training.

Speakers and presentations:

Pranav Kothari, MD — A national leader in health care delivery redesign, Dr. Kothari is the consultant to the Oregon Health Leadership Council's pilot project to improve patient outcomes and reduce costs for chronically ill patients.

Katherine Kendall, RN, IBCLC — As the Nurse Care Manager at Northwest Primary Care in Portland, Ms. Kendall works with a team to care for 200 high-risk patients as part of a medical home project. 

Charles Hofmann, MD, MACP — Dr. Hofmann successfully transformed a rural internal medicine practice into a patient-centric, team-based organization inclusive of all the clinic's staff.  Presentation

Rachel Solotaroff, MD — Dr. Solotaroff is the medical director for Old Town Clinic in Portland which has received national recognition for delivering effective integrated health care and social services.  Presentation

Michael Bleich, PhD, RN, FAAN — Dean of the School of Nursing at Oregon Health & Science University, Dr. Bleich is an expert in incorporating medical home principles in safety net clinics.

Sherril Gelmon, DrPH, FACHE — Chair of Public Administration and Professor of Public Health at Portland State University, Dr. Gelmon started her career as a physiotherapist and is an internationally recognized expert on the design and evaluation of educational programs for health service professionals.  Presentation

Priyesh Patel, PharmD, MBA — Dr. Patel, Chief Pharmacy Officer of Providence Health & Services Oregon, is leading the change in hospital pharmacy services from product to patient-centered models of care in clinical, operational and business areas.  Presentation

Heidi Brill, MPH — With first-hand knowledge of the role changes brought about by patient-centric care, Ms. Brill is an experienced Medical Assistant at the OHSU Richmond Clinic and recently graduated from OHSU with an MPH in Primary Care and Health Disparities.  Presentation

Tina Edlund, MS — An expert in health services research and policy, Ms. Edlund, Chief of Policy at the Oregon Health Authority, has a key leadership role in helping to transform Oregon's health care delivery system. Presentation
